Parents completing the form for applications for their child should palce their name in the "Name" field and include child's name and date of birth in the message area;   if more than one child, be sure to inlcude names and dates of birth of all children.  Minors completing form should put parent/guardian informaion  on the top area of the form and place their own name and date of birth in the message area.
